This is typically caused by marking a repeating task as complete more than once. Examples would include marking the task as complete both on a smartphone and tablet in between syncs, or marking it as complete once in the app and again in a widget before the widget refreshes. Each time a repeating task is marked as complete, a new task is created based on the repeating pattern.
